If you are a new patient that would like to register with Sanders Hand & Pediatric Therapy for either pediatric or hand therapy services, we have online and downloadable forms that you can print and fill out before visiting one of our clinics.

When initally contacting our recepetion staff, they will likely direct you to electronic registration forms. If that is not an option, please download and print new patient registration forms and bring them with you to your first visit.
